About the Kanagawa Prefecture Medical Collaboration Handbook (Cancer Regional Collaboration Critical Path)

In Kanagawa Prefecture, in cooperation with cancer treatment collaboration center hospitals, regional cancer treatment collaboration center hospitals, and Kanagawa Prefecture cancer treatment collaboration designated hospitals, we are operating the Kanagawa Prefecture Medical Collaboration Handbook, a regional collaboration critical path for the five major cancers (breast cancer, lung cancer, stomach cancer, colorectal cancer, and liver cancer) that can be used by all medical institutions within the prefecture.

*Note: For stomach cancer, two types of notebooks will be used depending on the condition.

The benefits of using a planner

This is used by cancer patients after undergoing treatment such as surgery, and is a notebook that compiles the patient's treatment plan for the next five to ten years.
By bringing this notebook with patients when they visit a medical institution, the base hospital or designated hospital where surgery or other procedures were performed, their family doctor, and other medical institutions can share information about the patient's treatment history, allowing for more appropriate medical treatment.
By having a medical record, patients will know when, where, and what kind of examinations and tests they should receive.

About using the notebook

After a patient who has received cancer treatment at our hospital has their doctor determine whether they would like to continue treatment at a local clinic or medical center, they will begin using the notebook after explaining the situation to the patient and their family and obtaining their consent.

After you start using the handbook

Your regular doctor will provide your daily medical care and prescribe medication when your symptoms have subsided. You will need to visit our hospital at key points in your treatment.
If you have any concerns, please consult your doctor. If you are unable to see your doctor in an emergency on a holiday or at night, please contact our clinic.

About Cancer Board

A cancer board is a conference (case study meeting) that brings together doctors and medical staff with specialized knowledge and skills in surgery, radiation therapy, and chemotherapy to decide on the appropriate treatment plan for each patient.
This is one of the requirements for designation as a cancer treatment collaboration center hospital, and its purpose is to exchange opinions, share information, consider and confirm things, etc., not only with the relevant department but also with various specialist staff in cases where it is difficult to determine a diagnosis or treatment plan.
In order to achieve the above objectives, our hospital has established a Cancer Board Management Subcommittee under the Cancer Treatment Collaboration Center Hospital Management Committee, which responds to case consultations as needed and brings together members of the subcommittee with doctors and medical staff with the specialized knowledge required for each case to help decide on treatment plans.
We also plan and hold lectures about four times a year to enable medical professionals to learn and share information about the latest papers and new initiatives.
