Hospitalization Information

Flow from admission to discharge

1. Outpatient visit

First, please be examined by a doctor. If the doctor determines that hospitalization is necessary, he or she will give you a hospital admission order.
At that time, you will be given a Guide to Admission Procedures, so please take it to the Admission/Discharge Center on the 1st floor or the Short-Term Inpatient Surgery Center on the 3rd floor.

2. Advance preparation

We will explain the procedures for admission to hospital.

Nurse interview

At our hospital, pre-admission interviews are conducted by nurses from the admission and discharge center in order to alleviate any worries or anxieties patients may have about their hospital stay and to start providing support for discharge as early as possible.


Patient Support Center Information

Doctors, nurses, pharmacists, medical social workers, and administrative staff work together to support patients so that they can receive safe and secure medical care, from the time they visit the hospital, through to their hospital stay and life after they are discharged.

Notification of admission date *Excluding closed days

Patients will be contacted by phone once their admission date has been confirmed.
(2. Patients whose admission date has already been decided at the time of admission reservation will be contacted only if there is a change.)

The day before admission *Excluding closed days

We will contact you by phone to inform you of the time to come to the hospital on the day of admission.

  • We will also conduct a simple health check to check for any infectious diseases (such as influenza) that may be present.

3. On the day of hospitalization

Please come directly to the specified time and place.
Admission procedures are carried out at each ward. Please bring the procedure documents, insurance card, limit application certificate, various medical vouchers, treatment card, seal, etc. given to you when you made your admission reservation.

  • If you are admitted to the pediatric ward, please also bring your maternal and child health handbook.

  • Patients who are admitted to rooms that incur a room charge are required to sign a consent form. Thank you for your understanding and cooperation.

4. Procedures after discharge

Discharge Procedures

Once your discharge has been decided, the ward office will give you a "Medical Expense Payment Confirmation Form." Please fill in the necessary information and submit it to the ward on the day of discharge.
There is no payment on the day of discharge, so you can go home right away.
(The invoice will be sent to you by mail at a later date.)

Outpatient follow-up visit after discharge

You will be given a reservation form when you are discharged from the hospital. Please come directly to the department on the day of your reservation.
