Our mission in the Department of General Internal Medicine (GIM) is to provide compassionate, patient-centered care while promoting high-quality health management and preventive medicine within the community. We are generalists proficient in emergency and acute care, capable of accurately triaging and managing a wide spectrum of medical conditions. Our physicians possess broad expertise across all medical disciplines and deliver care that integrates not only physical and laboratory findings but also psychological and functional aspects of health. With a comprehensive and holistic perspective, we provide cross-organ, system-based medical care. All members of our department strive to embody the ideals of excellence in general internal medicine and are dedicated to continuous professional growth and the delivery of advanced, high-quality medical services. Scope of Practice

Primary care, Infectious diseases, and Intensive Care Medicine

The Department of GIM primary a wide range of services encompassing primary care, emergency medicine, and infectious diseases. While university hospitals traditionally emphasize highly specialized, organ-based medical care, many illnesses are systemic in nature and often involve multiple organ systems. Our department addresses this gap by managing patients with common and undiagnosed diseases referred from local clinics, emergency outpatients, and those transported by ambulance requiring secondary or tertiary care. We also operate an Intensive Care Unit (ICU) staffed by physicians trained in critical care, managing severely ill patients transported by ambulance, those who deteriorate suddenly in the hospital, and patients referred from other institutions for advanced treatment. In addition, our infectious diseases team provides comprehensive care for patients with emerging infections such as HIV, as well as consultative services for hospitalized cases of infectious disease.

Major Conditions Managed

General Internal Medicine:

Syncope, loss of consciousness, dyspnea, chest or back pain, palpitations, headache, dizziness, fever, shock, acute abdominal pain, electrolyte imbalance, hyperglycemia, hypoglycemia, sepsis, endocrine disorders, anemia, disseminated intravascular coagulation (DIC), and other systemic conditions.

Medical ICU:

Shock, sepsis, multiple organ failure, severe pneumonia, respiratory failure, status asthmaticus, acute respiratory distress syndrome (ARDS), severe heart failure, acute renal failure, metabolic disturbances, diabetic ketoacidosis, severe electrolyte imbalance, and endocrine crises (adrenal or thyroid).

Infectious Diseases:

Emerging and re-emerging infectious diseases including HIV infection; travel-related infections (diagnosis, treatment, and prevention); and consultation on complex or refractory infectious diseases.

Clinical Activity

The department admits approximately 338 inpatients annually. Of these, around 58% are infectious disease cases, including sepsis, pneumonia, urinary tract infection, soft tissue infection, and infective endocarditis. Respiratory diseases account for about 12%, while the remaining cases include cardiovascular, renal, neurological, endocrine/metabolic, oncologic, hematologic, and collagen vascular diseases. In addition to daytime outpatient admissions, our department manages after-hours emergencies and patients brought in by ambulance, serving as the frontline for comprehensive internal medicine care. Our faculty members each contribute specialized expertise:

· Professor Hideki Ozawa – Cardiology · Associate Professor Masayuki Oki – Hematology · Lecturers Hidetaka Yanagi and Saki Manabe, and Assistant Professor Ayumi Tsuda – Infectious Diseases and Intensive Care The ICU team, led by intensive care specialists Drs. Yanagi and Manabe, manages critically ill patients with severe sepsis, shock, respiratory failure, circulatory collapse, and diabetic ketoacidosis. We also accept referrals for cases too complex to be managed at general hospitals.

Dr. Hidetaka Yanagi plays a central role in infectious disease care. As a major referral center for western Kanagawa Prefecture, we have extensive experience managing emerging infections, including HIV. Please feel free to refer patients with any infectious disease, not limited to HIV infection.
