Gastroenterological Surgery

Gastroenterological Surgery department strives to meet the needs of our patients and contribute to regional medical care. Our organ-specific divisions, consisting of upper gastrointestinal, lower gastrointestinal, and hepatobiliary, pancreatic, and rectal sections, provide highly specialized surgical treatment for malignant diseases. Our experienced staff also treat benign conditions, such as esophageal achalasia, reflux esophagitis, gastroduodenal ulcers, cholelithiasis, and inguinal hernias, as well as emergency transport. We receive many patient referrals from within and outside Kanagawa Prefecture, and our surgical experience for esophageal, gastric, colon, pancreatic, bile duct, and liver cancers is among the best in Japan. We also perform robot-assisted surgery using the Da Vinci system, which allows for minimally invasive and precise surgery, on many patients with esophageal, gastric, rectal, colon, pancreatic, and liver cancer, making us one of Japan's leading hospitals.

Our department's motto is "from diagnosis to treatment," so that we can provide responsible medical care to referred patients. Unlike so-called centralized facilities, we place great importance on the entire process from diagnosis to treatment. Cancer treatment involves many methods, including endoscopic therapy, surgery, chemotherapy, immunotherapy, and radiation therapy. Multidisciplinary treatments, such as pre- and post-operative chemotherapy (radiation) therapy, have also become standard. By providing these diverse treatments in Gastroenterological Surgery, we are able to provide patients with accurate and seamless care. Even if cancer recurs after surgery, we strive to alleviate the anxiety of patients and their families by being involved in the treatment.

Patient needs are becoming more diverse, and the number of elderly patients with underlying diseases and cancers in other organs is increasing. In response to this historical background, we always consider "what is best" for each patient and their family, and strive to provide medical care. Furthermore, for patients who are difficult to treat at other facilities, we take advantage of our university hospital's advantages and provide safe surgical treatment under a cross-sectional cooperative system within the hospital. We also participate in many clinical trials and clinical studies to create new and standard treatments, and quickly provide the latest knowledge gained from such multi-institutional collaborative research.

Treatment details

Gastroenterological Surgery aims to be a medical department that is trusted by doctors at referring medical institutions.

For malignant esophageal diseases, we combine endoscopic examinations, gastrointestinal contrast examinations, CT, MRI, PET/CT, etc., and provide appropriate treatment based on accurate diagnostic results. For benign esophageal diseases, we have a specialized testing system in place, including esophageal pressure tests and pH/impedance tests, and we accept even benign diseases that are difficult to treat. Endoscopic surgery using minimally invasive thoracoscopes and laparoscopes is the standard surgery, and we also actively perform robot-assisted surgery.

We perform surgery, endoscopic therapy, and chemotherapy for gastric diseases, primarily targeting gastric cancer and gastric submucosal tumors. Our treatments are characterized by preserving the stomach as much as possible and reducing surgical invasiveness, resulting in a significant improvement in postoperative quality of life. For early-stage gastric cancer, we actively perform laparoscopic robot-assisted surgery. For terminally ill patients, we work with Department of Palliative Care to provide home care with total parenteral nutrition.

We perform approximately 200 colon cancer surgeries per year. For colon cancer, we basically perform laparoscopic surgery, minimizing pain and suffering while maintaining the cure for the cancer, and aiming for early social reintegration of patients. We are also expanding the application of laparoscopic surgery for rectal cancer. In particular, for locally advanced lower rectal cancer, we perform preoperative chemoradiotherapy, which is the international standard treatment, to improve treatment outcomes. In addition, we will introduce laparoscopic surgery widely for recurrent cancer, malignant diseases other than carcinoma, inflammatory bowel diseases such as ulcerative colitis and Crohn's disease, chronic intestinal obstruction, small intestinal diseases, and emergency diseases, aiming to improve treatment outcomes. We also specialize in surgery for pelvic recurrence and advanced malignant diseases.

The Hepato-Biliary-Pancreatic Surgery Department primarily performs surgical treatment and chemotherapy for hepato-biliary-pancreatic cancer. We also treat benign diseases such as cholelithiasis. We perform minimally invasive endoscopic surgery using laparoscopy in many cancer cases, and also perform robot-assisted surgery. We work closely with Gastroenterology and Department of Diagnostic Imaging to perform diagnostic imaging and endoscopic diagnosis, and select the optimal treatment after discussions at joint conferences between the Surgery, Gastroenterology, Department of Diagnostic Imaging, and Department of Diagnostic Pathology. Thanks to excellent teamwork between each department and a proven track record of treatment by experienced surgeons, our hepato-biliary-pancreatic surgery department has been certified as a high-level training facility A by the Japanese Society of Hepato-Biliary-Pancreatic Surgery.

Main target diseases

1. Esophageal diseases:

Esophageal cancer, esophagogastric junction cancer, gastroesophageal reflux disease (GERD), hiatal hernia, esophageal achalasia, esophageal diverticulum, submucosal tumors of the esophagus, spontaneous esophageal rupture, etc.

2. Stomach disease:

Gastric cancer, gastric submucosal tumor (GIST, etc.), gastric ulcer, duodenal ulcer, etc.

3. Colon diseases:

Neoplastic diseases: Colon cancer, rectal cancer, colon polyps, polyposis, liver and lung metastases of colon cancer, recurrent colon cancer, pelvic recurrence, retroperitoneal tumors, GIST, NETs, etc.

Main medical achievements

1. upper gastrointestinal tract

In 2025, the total number of patients treated for esophageal diseases was 722, and the total number of patients treated for esophageal cancer was 417. There were 78 newly diagnosed cases of esophageal cancer in 2025, of which 47 underwent surgery, and 31 underwent endoscopic treatment, stent insertion, chemotherapy, or chemoradiotherapy. Regarding esophagogastric junction cancer, which has been increasing recently, surgery was performed on 13 cases. Additionally, surgery was performed on 11 cases of benign esophageal diseases, including submucosal tumors, gastroesophageal reflux disease, achalasia, esophageal diverticulum, and spontaneous esophageal rupture.

Thoracoscopic surgery, laparoscopic surgery, and robot-assisted surgery, which involve smaller incisions and less pain, are collectively called endoscopic surgery and are performed on approximately 70% of esophageal cancers nationwide. Our department is a leading facility in Japan and performs these procedures on almost all cases. We introduced robot-assisted surgery in 2019, and in 2025, we performed 41 robotic surgeries for esophageal cancer. Our department is a "certified facility for esophageal surgery specialists" by the Japan Esophageal Society and a "certified participating facility" of the Japan Clinical Oncology Group (JCOG), contributing to the provision of advanced medical care for esophageal diseases and the establishment of standard treatments.

Our hospital primarily treats gastric diseases, including gastric cancer and submucosal tumors (GIST, etc.). According to our 2025 hospital cancer registry, we performed 275 gastric surgeries, 96 of which were surgical procedures. Chemotherapy (anti-cancer drug treatment) is administered to unresectable, metastatic, or recurrent gastric cancers. Surgical treatment for gastric cancer involves determining the resection method based on the location and depth of the cancer, and the presence or absence of metastasis (lymph node metastasis and distant metastasis). Options include distal gastrectomy, total gastrectomy, and proximal gastrectomy. Partial gastrectomy is performed for gastric submucosal tumors. Surgical approaches include open surgery, laparoscopic surgery, and robot-assisted surgery, selected according to the patient's condition. In recent years, we have been actively performing minimally invasive surgeries (robot-assisted surgery and laparoscopic surgery) that are less burdensome for patients, and the number of these procedures is increasing. In 2025, there were 20 robot-assisted surgeries and 58 laparoscopic surgeries. Chemotherapy is administered for unresectable or metastatic/recurrent gastric cancer. For patients with Stage II/III gastric cancer diagnosed after surgery, adjuvant chemotherapy is administered to prevent recurrence. For GIST treatment, molecular targeted therapy is used in addition to surgery. Furthermore, our hospital meets the criteria set by the Japanese Gastric Cancer Association regarding the number of specialists, clinical performance, and academic achievements, and is certified as a Japanese Gastric Cancer Association Certified Facility A, working in collaboration with pathologists and radiologists to provide medical care.

2. lower gastrointestinal tract

We treat approximately 150 cases of colon cancer and 60 cases of rectal cancer each year. Colorectal cancer is classified into stages I, II, III, and IV depending on its progression. With surgery alone, recurrence occurs in 5% of stage I cases, 20% of stage II cases, and 35% of stage III cases. In stage IV, treatment for metastases to the liver, lungs, etc., must be carried out simultaneously. Therefore, we provide specialized treatment (called multidisciplinary treatment) tailored to the progression of each individual patient's cancer. Specifically, our department's characteristics include: ① using robotic or laparoscopic surgery for cancers other than large tumors and cancers that have invaded surrounding tissues; and ② combining chemoradiotherapy before surgery for locally advanced rectal cancer. This reduces the pelvic recurrence rate, which is as high as 15-20% with surgical treatment alone, to 2-3%, and further shrinks the cancer to avoid a permanent colostomy. We also specialize in surgery for pelvic recurrence and highly advanced malignant diseases. In 2025, we performed 178 cases of colorectal cancer resection (113 cases of colon cancer, 68 cases of rectal cancer), 10 cases of recurrent colorectal cancer resection, 36 cases of other malignant disease resection, and 177 cases of benign disease surgery (8 cases of inflammatory bowel disease, 12 cases of anal disease, 41 cases of inguinal hernia surgery, and 116 other cases). Recently, reports have emerged, mainly in Europe and the United States, that combining radiotherapy and chemotherapy (TNT: Total Neoadjuvant Therapy) for advanced rectal cancer eliminates the need for surgery in approximately 30-50% of patients, and we have also introduced this treatment in our department. To date, we have performed TNT on more than 40 patients, and nearly 40% of them are being monitored without surgery. In order to fulfill our mission as a university hospital, we also participate in various clinical trials and studies. We also focus on mentoring young doctors.

3. Liver, gallbladder and pancreas

Each year, we perform approximately 60 pancreatic resections (including laparoscopic distal pancreatectomy and robot-assisted distal pancreatectomy) and approximately 60 liver resections (including laparoscopic liver resection and robot-assisted liver resection). Since April 2024, we have established Pancreatic and Biliary Diseases Cente, where the Departments Gastroenterological Surgery, Gastroenterology, Department of Diagnostic Imaging, Department of Radiation Therapy, Department of Diagnostic Pathology, and Clinical Oncology hold weekly joint conferences, focusing particularly on diagnosis, treatment, and collaborative research in this field. In addition to performing numerous highly complex pancreatic and liver resections, we actively strive to improve treatment outcomes by implementing preoperative and postoperative adjuvant chemotherapy. Cancers in the hepatobiliary and pancreatic region are often difficult to treat, but in recent years, advances in drug therapy (anti-cancer drug treatment) have increased the number of drugs that can be used under health insurance. Even for cancers that are deemed difficult to resect at the initial consultation, it may be possible to present a "treatment strategy that does not give up on resection" by administering anti-cancer drug treatment first. Particle beam therapy for locally advanced cancers is also covered by insurance, and from a surgical perspective, we present all of these treatment options and provide treatment methods that maximize the benefits of surgical treatment. Furthermore, as most patients are now elderly, we collaborate with local medical institutions to provide long-term multidisciplinary treatment that utilizes exercise therapy, nutritional therapy, and supportive care during neoadjuvant chemotherapy, so that the patient's overall condition and tumor state before surgery are in a state that allows for the best possible surgical treatment effect.
