肝疾患医療センター

ご挨拶

肝臓病はまだ多くの患者さんが苦しんでいる病気ですが、近年、新しい治療薬が次々に登場し、これまで治療困難であった病気をコントロールすることが可能になってきています。当センターでは地域の医療従事者と共に、患者さんの言葉に真摯に耳を傾け、患者目線にたった情報、治療を提供していきたいと思っています。

Liver diseases continue to affect many patients, and some still face serious challenges despite advances in medical treatment. In recent years, however, new therapeutic options have emerged one after another, making it possible to control conditions that were once difficult to treat.

At our center, together with healthcare professionals in the community, we are committed to listening sincerely to each patient and providing information and treatment from a truly patient-centered perspective.

概要

ウイルス性肝炎は日本の国民病と言われるほど患者数が多く、放置すると肝硬変、肝癌に移行することが知られています。平成22年1月に肝炎対策基本法が施行され、肝疾患診療の均てん化・医療水準の向上を目的として、各都道府県に肝疾患診療連携拠点病院を設置し、医療従事者、一般市民に対して日々進歩する肝疾患治療に関する情報を提供することになりました。当院は神奈川県にある4つの拠点病院のうちの一つであり、具体的に肝炎対策業務を遂行するのが当センターです。
肝疾患医療センターは「就労を維持しながら適切な肝炎医療を受けることができる環境の整備」を目的として、肝疾患に関する診療ネットワークの整備、肝疾患診療に関わる人材の育成、肝疾患に関わる相談支援および情報提供を行っています。

構成員

当センターは医師3名、看護師2名、管理栄養士1名、薬剤師1名、ソーシャルワーカー2名、事務3名の計12名で運営されています。

業務

  • 地域の医療従事者を対象とした研修会の開催

  • 地域の医療従事者との肝疾患パスを用いた医療連携

  • 肝炎患者、家族、地域住民を対象とした肝疾患講演会の開催

  • 肝炎患者やその家族を対象とした肝臓病教室の開催(肝臓専門医、看護師、管理栄養士、薬剤師による生活指導、栄養指導)

  • 肝炎患者やその家族からの相談への対応

  • 肝炎訴訟におけるカルテ開示手続き

Overview of the Center

Viral hepatitis has long been considered a major public health issue in Japan, affecting a large number of people. If left untreated, it can progress to liver cirrhosis and liver cancer.

In January 2010, the Basic Act on Hepatitis Measures was enacted with the aim of ensuring equal access to liver disease care and improving overall treatment standards.
Under this initiative, designated Liver Disease Core Hospitals were established in each prefecture to provide up-to-date information on liver disease to both healthcare professionals and the public.

Our hospital is one of the four core hospitals in Kanagawa Prefecture, and this center is responsible for implementing hepatitis-related programs.
The mission of the Liver Disease Medical Center is to ensure that patients can receive appropriate treatment while maintaining their daily lives and employment.

To achieve this, we:

  • Build regional medical care networks

  • Train healthcare professionals involved in liver disease

  • Provide consultation and support to patients and families

  • Disseminate accurate and accessible information on liver conditions

Multidisciplinary Team

The Liver Disease Medical Center is operated by a dedicated multidisciplinary team consisting of:

  • 3 Physicians (Liver specialists)

  • 2 Nurses

  • 1 Registered Dietitian

  • 1 Pharmacist

  • 2 Medical Social Workers

  • 3 Administrative Staff

Together, we provide comprehensive medical care, lifestyle guidance, and social support for patients with liver disease and their families.

  • Training seminars for regional healthcare professionals
    Promoting collaboration and improving standards of liver care within the community.

  • Medical coordination using liver disease care pathways
    Strengthening cooperation between hospitals and clinics for long-term management of liver patients.

  • Public lectures for patients, families, and local residents
    Sharing the latest information on hepatitis and liver diseases.

  • Liver disease education classes
    Conducted by hepatologists, nurses, dietitians, and pharmacists, offering lifestyle, nutrition, and medication guidance.

  • Consultation services for patients and families
    Addressing concerns related to medical care, daily生活, and employment affected by chronic liver disease.

  • Support in hepatitis-related legal cases
    Assisting with procedures such as disclosure of medical records.
