Greetings

For patients, the minimum requirement for hospitalization is to receive high-quality medical care, even if only for a short period of time, and to be able to spend their hospital stay without worrying about post-discharge anxiety. Furthermore, strengthening regional collaboration has been a recent trend in national healthcare policy. Against this backdrop, the Patient Support Center was established to provide comprehensive support for patients referred from local medical institutions, from consultation to admission and discharge. The center is primarily responsible for "Patient Flow Management (PFM)," and its operations include the Admission/ Patient Admission and Discharge Office, staffed by nurses who optimize admission and discharge coordination; General Consulting Office, staffed by MSWs who provide medical consultations, support for reintegration into society, and hospital transfer coordination; Regional Medical Collaboration Office, staffed by medical affairs staff who coordinate appointment scheduling and reverse referrals for referred patients; Volunteer Office, which supports volunteer activities; and the Medical Office of Medical Mediators, staffed by medical mediators who facilitate dialogue between patients' families and medical professionals. We hope to continue deepening our ties with local medical institutions. All of our staff are committed to working hard, so we appreciate your continued support.

Overview

Patient Support Center, which includes General Consulting Office, Volunteer Office, Regional Medical Collaboration Office, Patient Admission and Discharge Office, Transplantation Support Service Center, and Office of Medical Mediators works together to support patients and their families, from outpatient visits and hospitalizations to discharge and transfers, in cooperation with local medical institutions, facilities, and related organizations. We provide support for the issues (medical, financial, psychological, and social) that patients and their families face, and are staffed with doctors, nurses, medical social workers, registered dietitians, pharmacists, administrative staff, and medical mediators. We work together to provide the necessary support to resolve and alleviate the problems faced by patients and their families.
