Breast Surgery

Greetings

Our department meets the requirements of the Japan Breast Cancer Society and the Japan Breast Cancer Screening Association as an institute that performs detailed examinations, and performs surgery on approximately 300 breast cancer patients per year. We also treat patients with advanced or recurrent breast cancer who are referred from all over the country. Our department practices evidence-based medical care, and is involved in the creation of Japan's breast cancer treatment guidelines. We are pioneers in immunotherapy and molecular targeted therapy, and are actively involved in the development of new drugs and medical techniques.

Treatment plans for patients are decided through joint conferences with the pathology, radiology, and ultrasound departments. Various prognostic factors are analyzed from the collected cancer tissue using immunohistochemistry and molecular biology methods. Then, in consultation with the patient and their family, adjuvant therapy is administered to prevent recurrence. In addition, for patients with recurrence, we formulate aggressive treatment strategies and have achieved good treatment outcomes.

Treatment details

Our department treats both malignant and benign breast diseases, with five breast specialists providing breast cancer care. We also have three cancer chemotherapy specialists on staff, who provide chemotherapy for other malignant tumors. We actively provide physical and psychological palliative care for patients and their families from the time of diagnosis of malignant disease through to the terminal stage, striving to provide medical care that allows patients to receive cancer treatment with peace of mind. We also work in collaboration with Department of Genetics to handle familial tumors and cancer genome testing.

<Mammary gland disease>
Specialization is important in the diagnosis and treatment of breast cancer, and the content is improving every day. Our hospital plays a central role in this field in the western Kanagawa region. The breast disease treatment policy is centered on breast specialists, and we hold joint conferences every week with pathologists, radiologists, certified radiologists, and ultrasound technicians to provide team medical care.

<Diagnosis>
If necessary, we aim to provide a prompt diagnosis at the first visit by performing mammography, breast ultrasound, cytology, and tissue biopsy.

<Surgery/post-operative therapy>
Before surgery, we inform the patient of the disease, perform a high-resolution breast MRI scan to determine the appropriate extent of resection, explain the advantages and disadvantages of breast-conserving surgery and mastectomy, and present the available surgical procedures to the patient for selection. From detailed intraoperative and postoperative pathological examinations to scientifically based aggressive postoperative drug therapy, we provide everything on an outpatient basis. Furthermore, if breast reconstruction is desired, we work with plastic surgery to perform rectus abdominis and latissimus dorsi flap reconstructions and implant reconstructions.

<Treatment for advanced and recurrent breast cancer>
For the treatment of advanced or recurrent breast cancer, we provide tailor-made treatment that takes into account the wishes and circumstances of each individual patient, while also adhering to the latest evidence-based principles.

We also participate in development trials of new drugs and many clinical trials on a global scale, giving us more treatment options than other facilities and striving to improve treatment outcomes for cases in which existing treatments are ineffective.

<Cancer Genomic Medicine>
Cancer genomic medicine is a type of personalized medicine that uses the patient's cancer cells to examine genetic changes and provide treatment tailored to the characteristics of each individual's cancer. Our department at our hospital is involved in cancer genomic medicine for various malignant tumors, not just breast cancer, and we select and propose treatments that are tailored to the characteristics of each patient's cancer.

Main medical achievements

Our department treats breast diseases ranging from malignant to benign, with a particular focus on breast cancer. In 2024, we performed 358 surgeries (326 breast cancer surgeries under general anesthesia). We performed 124 breast-conserving surgeries, 202 mastectomies (30 breast reconstructions), and 282 sentinel lymph node biopsies. The number of new outpatients was 626. At the outpatient chemotherapy center, which is comfortably equipped, we provide safe, reliable, and prompt treatment to a total of 2,961 patients by dedicated doctors, nurses, and pharmacists. In 2024, we accepted 27 clinical trials, and many clinical trials are currently underway, especially for advanced and recurrent breast cancer and luminal early breast cancer.
